MD Revolution has acquired remote patient care services provider NavCare for expanding remote patient care management services.
The company offers remote patient monitoring (RPM) and chronic care management (CCM) services to 150,000 patients and more than 1,700 providers.
Located in Augusta, Georgia, NavCare is a clinician-led care centre that offers virtual CCM, telehealth care management, and RPM.
It offers preventative and proactive health screenings, assessments, and vital tracking services using connected remote monitoring devices, the NavCare Connect integrated health platform, and a 24/7 care team.
Financial terms of the transaction were not revealed.
MD Revolution CEO Kyle Williams said: “NavCare’s experience in hospitals and its track record with annual wellness visits (AWV) and transitional care management (TCM) aligns with our growth strategy.
