Indian technology services company Persistent Systems, in collaboration with Microsoft, has introduced a new generative artificial intelligence (AI)-driven population health management (PHM) solution.
This solution is designed to improve patient care and optimise healthcare providers’ capacity and cost-effectiveness.
The PHM Solution, which is in line with value-based care models, uses Social Determinants of Health (SDoH) to better predict healthcare costs and identify non-clinical patient needs.
A key feature of this solution is predictive risk modelling, which helps in identifying patients in need of significant medical support.
Persistent’s PHM Solution leverages Microsoft Azure OpenAI Service and Dynamics 365 Customer Insights to extract SDoH from electronic health records (EHR) data.
According to Persistent Systems, approximately 80% of information available within EHRs is in the form of unstructured clinical notes and its PHM solution can accurately extract key patient data, unlike its counterparts in the market, which depend on third-party data sources or survey data.
