The West Virginia University Health System, known as WVU Medicine, has formed a strategic partnership with Interwell Health to transform care for patients with kidney disease in West Virginia, US.
Kidney care management company Interwell Health works in collaboration with physicians, government, and private payers to deliver value-based care.
The company said its approach has been shown to enhance clinical outcomes, improve patients’ quality of life, and reduce overall healthcare costs.
With a provider network exceeding 1,800 nephrologists across the US and Puerto Rico, Interwell Health supports more than 125,000 patients in value-based care agreements.
As part of the latest agreement, WVU Medicine’s nephrologists will join the expansive Interwell Provider Network and adopt its population health model for kidney care.
This model includes a holistic care team of nurses, dietitians, care coordinators and social workers, who provide continuous support and education to patients, aiming to slow the progression of kidney disease.

Interwell Health cited the Centers for Disease Control and Prevention’s findings showing West Virginia as having the nation’s highest mortality rate for individuals suffering from CKD [chronic kidney disease].
WVU Medicine nephrologist Rebecca Schmidt said. “By approaching kidney disease from a population health perspective, we can use machine learning and other technologies to examine the progression of the disease and use that data to implement more effective treatment plans.
“We understand that progress cannot happen in a vacuum, and Interwell presents the opportunity to collaborate with others on best practices and use this collaboration to help better the lives of West Virginia patients with kidney disease.”
WVU Medicine encompasses 21 hospitals, including the J W Ruby Memorial Hospital and the WVU Medicine Children’s Hospital, and operates with more than 3,000 licensed beds and 4,000 providers.
