US-based healthcare organisation WakeMed has partnered with healthcare technology company Bamboo Health in an effort to enhance care collaboration and improve patient outcomes.
Bamboo Health is a real-time care intelligence provider that offers actionable insights on patients’ behavioural, physical and social health.
WakeMed will integrate Bamboo Health’s solutions at its three acute care hospitals, eight emergency departments and numerous physician practices.
The technology will offer real-time admission, discharge and transfer (ADT) e-notifications, alerting providers to patients’ statuses across various healthcare settings.
WakeMed’s use of these solutions aims to ensure seamless care for the company’s patient population, which includes Medicaid, Medicare, commercially insured and uninsured individuals.
Based in North Carolina, WakeMed is a not-for-profit health care system with a bed capacity of 970.

“Now our care managers and providers will be alerted at the point of care about care events and community relationships that were previously invisible—those happening outside of WakeMed—so they can intervene to help patients before they end up in the emergency department, get them the right care once they’re here, and ensure they receive appropriate care after they leave.”
Bamboo Health’s Pings solution will also be integrated into WakeMed’s clinical workflow.
The solution draws ADT data from a national network to allow rising-risk patients to be identified quickly, enabling timely interventions that could lead to improved outcomes and cost savings.
