Health information network Surescripts is expanding its Medication History for Populations solution to health plans in an effort to support patient care, improve medication adherence and reduce costs.

The initiative aims to provide health plans’ care management teams with essential medication data to identify key opportunities for addressing gaps in care and increasing adherence to medication.

Surescripts said the Medication History for Populations solution contributes to positive Healthcare Effectiveness Data and Information Set (HEDIS) measures and Star Ratings that health plans strive to achieve.

It is intended to simplify patient identification among at-risk populations who may benefit from outreach regarding their medications.

The solution is also designed to keep patients healthy over time by monitoring their medication activity, as well as offer care managers an overview of patients’ health.

More than a quarter of US adults are facing multiple common chronic conditions such as diabetes and hypertension, while the population among those aged 65 and older is rapidly increasing.
