Vesta Healthcare, a provider of home-based healthcare solutions, has partnered with Medical Guardian to offer an integrated solution for chronic care management and remote patient monitoring (RPM).
The partnership aims to reduce hospitalisations for Medicaid and Medicare members by combining 24/7 clinical oversight of Vesta Healthcare with advanced monitoring devices of Medical Guardian.
Medical Guardian’s monitoring devices include cellular-connected vitals monitoring devices and personal emergency response systems (PERS).
The devices are user-friendly and the integration of RPM and PERS allows for real-time health monitoring in the home setting.
With the assistance of these devices, Vesta Healthcare’s clinicians collect health information, including blood pressure, weight, oxygen levels, and fall detection.
