Habitat Health, in collaboration with Kaiser Permanente, has inaugurated its first PACE centre at 855 Howe Avenue in Sacramento, US.
The facility is designed to provide older adults with a range of services, including primary and speciality care, social activities, prescriptions, and transportation, in one location.
Participants in the PACE programme, who are typically eligible for Medicare and Medi-Cal, can access these services at no cost.
Habitat Health with Kaiser Permanente Sacramento vice-president and general manager Brad Oglevee said: “We are excited to offer a truly differentiated care experience to support the diverse needs of older adults who live here in Sacramento.
“We know that bringing together Kaiser Permanente’s demonstrated expertise in delivering complex care with Habitat Health’s dedicated interdisciplinary teams and joyful activities here in our centre will provide older adults and their caregivers peace of mind and a sense of community.”
With its origins in California dating back to the 1970s, the PACE model has been associated with reduced hospital admissions, the alleviation of depression symptoms, and improvements in the well-being of elderly people and their caregivers. It also addresses health disparities.
