US-based non-profit healthcare system BayCare has partnered with Cadence, a company specialising in chronic disease management, to launch a remote patient monitoring and responsive virtual care programme.
The initiative aims to extend care to the homes of patients with various chronic conditions such as type 2 diabetes, congestive heart failure and hypertension, in Florida’s Tampa Bay and West Central regions.
It will leverage BayCare Medical Group’s clinical expertise and Cadence’s technology platform to offer proactive and preventive healthcare.
The partnership will allow patients enrolled with Cadence to have vital signs such as glucose levels, weight, heart rate and blood pressure monitored in real-time.
This data can then be shared with the patient’s primary care physician and Cadence’s round-the-clock clinical care team.

Cadence and BayCare clinicians will also work together to provide consistent monitoring, lifestyle guidance and medication optimisation, making interventions if any patients’ vitals are out of the normal range.
Based in New York, Cadence works with academic medical centres and health systems across the US to offer guideline-directed, life-extending medical therapy and care to patients.
The company’s nurse-practitioner-led care team supports more than 25,000 patients and over 1,200 providers across 22 US states.
