Cloud DX has signed a contract with the Provincial Health Authority of Prince Edward Island, Health PEI, for remote patient monitoring (RPM).
As per the agreement, Prince Edward Island will set up Connected Health RPM of Cloud DX to support primary care, home care and rehabilitation patients.
These are the three crucial healthcare use cases in the province.
The new contract is the second of its kind deal signed by Cloud DX after the company became the only RPM services vendor through Mohawk Medbuy Master agreement signed in June last year.
With the new RPM deployment in the province, primary care patients having chronic ailments such as chronic obstructive pulmonary disease (COPD) and congestive heart failure (CHF) will have access to Connected Health for boosting outcomes.
Additionally, home care patients with chronic diseases, who are on geriatric or palliative care, will be given Connected Health kits and services to cut down emergency room visits and admission in the hospital.

By GlobalDataCloud DX Connected Health telemedicine and vital sign monitoring will enable rehabilitation patients to benefit from improved care at home with focused nursing care.
The deployment of Cloud DX Connected Health will involve replacing a prior vendor.
The agreement for Connected Health Kits and remote care monitoring services is planned at supporting up to 300 new patients per year in the province.
With up to two possible extensions of two years each, the new contract has an initial term duration of three years.
The deal is valued at nearly C$1.34m ($1m) over the complete extended term, with expected expenditure for supplying the kits and services totalling C$295,000.
Apart from Health PEI, Cloud DX is the sole vendor of RPM services under three provincial-level agreements with Alberta Health Services, Alberta Primary Care and Network Yukon Health.
Health PEI Community Health and Seniors Care executive director Andrew MacDougall said: “Health PEI evaluated the Cloud DX Connected Health platform as part of the Mohawk Medbuy Request for Proposal (RFP) process prior to awarding this contract.
“Some features of Cloud DX Connected Health that really stood out and differentiated the solution include Cloud DX’s support for ease of deployment, patient onboarding, compliance to care instructions and patient technical support.
“These services are expected to increase efficiencies in delivering care throughout the province.”
