Collaborative Bridges, an alliance of safety net hospitals and mental health agencies in West Side Chicago, has partnered with HealthEC, a provider of population health management solutions, to improve care coordination for behavioural health patients. 

As part of this collaboration, HealthEC will provide its data and analytics solution to facilitate the integration of the electronic health records (EHR) and workflows of Collaborative Bridges’ organisation.

The move aims to enable the delivery of seamless care and planning services for individuals post-hospitalisation for behavioural health issues. 

HealthEC’s technology will enable Collaborative Bridges’ care coordinators to efficiently connect with patients after discharge, a time considered critical due to the heightened risk of suicide within the first week.  

By December this year, HealthEC plans to launch a system that merges clinical, claims, and health information exchange data, incorporating social determinants of health to provide care coordinators with actionable insights.
