US-based kidney care management company Interwell Health has introduced Acumen Rounder, a nephrology rounding application designed to enhance value-based kidney care management.
The solution is the first nephrology rounding application that integrates seamlessly with Acumen Epic Connect, which is claimed to be the world’s most adopted nephrology-specific electronic health record system.
It is designed to support nephrologists and care teams in managing patient workflows efficiently and accurately, as well as address the evolving needs of nephrologists in value-based care settings.
Acumen Rounder offers a comprehensive suite of features for nephrologists, including patient tracking, hospital follow-up, transitional care management (TCM), clinical documentation entry, predictive text for diagnoses, and a modern interface.
These features are intended to improve patient outcomes and user experiences for nephrologists in hospitals and dialysis centres.
One significant feature is its transitional care management capability, which aims to reduce readmissions by prompting physicians to arrange follow-up care within a critical 14-day period post-discharge.

As well as integrating with Acumen Epic Connect, the application also connects with the clinical networks of at least two leading dialysis companies.
It is set to fully replace Interwell’s legacy app, Acumen Mobile Charge Capture, projecting an uptake by 2,340 providers across 138 nephrology practices by the end of this year.
Earlier this year, Interwell Health partnered with the West Virginia University Health System (WVU Medicine) to improve care for kidney disease patients in West Virginia.
