Chronic care management and proactive care company Jaan Health has announced a partnership with NeuroNet to enhance chronic care management in neurology.
This collaboration is set to empower over 1,000 providers from 80 neurology practices in the US, enabling them to utilise Jaan Health’s Phamily platform to deliver proactive care more efficiently.
Furthermore, this approach is aimed at boosting patient satisfaction, and receive appropriate compensation for between-visit patient care.
The AI-driven learning loop and extensive dataset of Phamily are designed to facilitate two-way text-based communication, which has been shown to enhance patient outcomes and engagement.
The platform, supported by expert assistance, allows medical groups, health systems, and Accountable Care Organisations (ACOs) to engage with patients ten times more effectively compared to traditional care management methods.
On average, Phamily is claimed to achieve a patient engagement rate of over 70%, with patients interacting with their care teams on an average of 3.4 times per month.
