US-based healthcare providers UnitedHealthcare, Centura Health and Colorado Health Neighborhoods have partnered to enhance patient care in Colorado.
The collaboration will enable nearly 17,000 participants enrolled in UnitedHealthcare’s employer-sponsored health plan, to benefit from improved care coordination and better health services through Colorado Health Neighborhoods.
As part of the partnership, a new accountable care organisation (ACO) will be formed to improve quality and outcomes for patients, while reducing costs.
The ACO will allow primary care physicians to receive regular patient updates, monitor all of the care that each patient receives, and maintain complete clinical information about each patient.
Colorado Health Neighborhoods executive director John Suits said: "Colorado Health Neighborhoods is pleased to enter into a population health management opportunity with UnitedHealthcare.

The partnership will ensure that primary care physicians, specialists and facilities are aligned with the Institute for Healthcare Improvement’s ‘Triple Aim’ objective that calls for increasing patient satisfaction, improving health of the population and reducing health care cost.
