Virtua Health in the US has partnered with Unite Us to connect patients with essential community support services and improve care coordination.

The integration of the Unite Us platform within Epic will support clinicians and social workers to connect patients to a range of internal and third-party programmes and services.

The partnership between Virtua Health and Unite Us promotes health equity, as it allows referrals and outcomes to be tracked.

This enables Virtua Health to create personalised care plans with a wide spectrum of quality-of-life issues.

Services that patients can be connected to include mental and behavioural health counselling, food and nutrition aid, housing support, and financial assistance.

Unite Us co-founder and CEO Dan Brillman said: “This collaboration brings together two innovators with a shared commitment to improving health outcomes through proven technology and community-driven solutions.”

Once implemented, more than 3,000 providers across 400 care locations within the Virtua Health network will use Unite Us to standardise the Health-Related Social Needs (HRSN) referral process.

This initiative aligns Virtua Health with other health systems and government agencies that are part of the Unite New Jersey coordinated care network.
