US-based health technology company WellSky has launched the WellSky CarePort Care Transitions Dashboard Suite, a tool designed to enable data-driven care decisions.

The suite of dashboards aims to provide acute care providers with enhanced insights into referral metrics and trends through ‘intuitive’ visualisations.

By integrating data from WellSky’s CarePort solutions, electronic health records (EHR) and claims, the dashboards are designed to improve operational efficiency.

The suite’s interactive features and drill-down capabilities are designed to enable a comprehensive exploration of data by various criteria, such as level of care, service type, or length of stay.

These allow clinicians to track and visualise key performance indicators, which can be customised to the needs of each organisation.

One of the dashboards is the WellSky Clinical Performance dashboard, which WellSky developed in collaboration with consulting firm The Center for Case Management (CFCM)

This dashboard aims to help hospital leaders analyse case management performance, identify areas for improvement, and implement strategies that could improve patient results.

Earlier this year, WellSky introduced a resource management solution that focuses on improving care delivery in the home health and hospice markets.
