US-based healthcare company Astrana Health has partnered with Elation Health, a clinical technology company, to enhance value-based care for primary care providers.
The partnership will focus on scaling risk-bearing entities such as accountable care organisations and clinically integrated networks, supported by affiliate providers.
Elation will contribute its knowledge of practice transformation and billing technology, as well as its electronic health record (EHR) system, which is currently used to provide care for 15 million patients worldwide.
As the exclusive care enablement partner, Astrana will offer its value-based care platform and solutions, as well as its care and disease management platforms and administrative services.
The partnership's focus is underscored by Astrana's agreement to provide its Care Enablement platform to a provider organisation in Hawai'i.
This initiative will integrate Elation's EHR technology with Astrana's infrastructure and care management capabilities, serving as a model for future collaborations.

Astrana and Elation will jointly support more than 100 primary care providers in Hawai'i, managing the care of more than 20,000 Medicare patients.
Providers are expected to join Astrana's Care Enablement platform by the end of the third quarter of this year.
