Biofourmis and Lee Health to introduce home-based hospital care model

The partnership with Biofourmis will support Lee Health's RPM service line.

Biofourmis, a global care delivery company, and Lee Health, a health system with five hospitals in Southwest Florida, have collaborated to introduce a Hospital at Home programme and extend their remote patient monitoring (RPM) programme.

This initiative aims to provide hospital-level care in patients' homes, improving satisfaction and clinical outcomes.

Lee Health, known for its RPM services, is attempting a 50% reduction in 30-day readmissions and maintaining an average daily census of over 700 RPM patients.

Moreover, the partnership will see the Hospital at Home programme offer an alternative inpatient care setting that brings resources, supplies, and care teams directly to patients' homes.

The Biofourmis platform offers continuous monitoring with features like integrated personal emergency response, arrhythmia and fall detection, and vitals monitoring, all accessible to the command centre nursing team 24/7.

Lee Health chief nursing officer Jennifer Higgins said: “Improving patient experience and supporting an environment that best promotes a patient’s health and well-being is of utmost importance for Lee Health.

“The Hospital at Home model utilises technology at a new level, giving us the chance to allow patients to heal in their own home with support from our compassionate staff of nurses and doctors.”

Biofourmis is also integrating with Lee Health's Epic Electronic Health Record (EHR) system to ensure seamless acute and post-acute care management and patient data accessibility.

The platform's FDA-cleared predictive analytics help reduce alarm fatigue and improve nursing efficiencies by identifying early signs of patient deterioration.

The programme's coordination and tracking capabilities aim to enhance the management of patient care and RPM logistics.
