Citizens Memorial Hospital in Missouri, US, has partnered with social care technology provider Unite Us to integrate the Meditech Expanse electronic health record (EHR) with a referral system.
The integration with Unite Us’ closed-loop referral system is intended to identify and address the social care needs of Citizens Memorial Hospital's patient population.
Clinical providers at the hospital will also be able to access patients' social care histories directly within Expanse.
Through this, Citizens Memorial Hospital aims to build and scale a coordinated care network, track outcomes, address gaps in services and monitor populations at risk.
The hospital was reportedly the first to adopt the Unite Us platform within Meditech Expanse, integrating it successfully into its operations.
Citizens Memorial Hospital CEO and executive director Michael Calhoun said: “With the integration in Expanse, we can more efficiently identify and address social care needs at the point of care.
“It also provides a more comprehensive view of our community’s overall health, helping us better prioritise initiatives that align with our patients’ needs.”
The Unite Us platform connects patients with community-based resources via the Unite Missouri network.
It is designed to serve as a central point for health and social service providers to securely access and refer PATIENTS to the services they require, while also tracking progress and assessing outcomes.
Located in Bolivar, Citizens Memorial Hospital provides care services for patients across eight counties in south-west Missouri.
