Cleveland Clinic expands Hospital Care at Home offering to Weston

Most admissions to the programme come directly from the emergency department, bypassing the hospital wards entirely.

American medical centre Cleveland Clinic has broadened its Hospital Care at Home programme, now offering services to Weston, Florida.

Initially introduced at Cleveland Clinic Indian River Hospital in Vero Beach in April last year, the offering was subsequently extended to Cleveland Clinic Martin Health and will now be expanded to Cleveland Clinic Weston Hospital.

The Hospital Care at Home initiative allows patients to access acute hospital-level care in an at-home setting.

In its first one-and-a-half years, the programme is said to have notably reduced hospital readmissions and aided over 1,800 patients in recovering at home.

This approach uses technology to keep patients constantly connected to their healthcare providers.

Doctors and nurses from the Cleveland Clinic closely monitor patients and are readily accessible at the touch of a button, the centre noted.

In addition, support staff aid in the delivery and set up of tablets and other necessary technology at the patient's home as soon as they return, facilitating immediate communication with their care team.

Patients suffering from conditions such as congestive heart failure, kidney infections, and pneumonia can now receive care at home.

Cleveland Clinic Weston Hospital vice-president and chief medical officer Scott Ross said: “We’re excited to offer our patients the same level of hospital care Cleveland Clinic is known for in the comfort of their own homes.

“Hospital Care At Home is bringing a new model of care to the community and will revolutionise how we view the hospital.”

In June this year, Cleveland Clinic Cleveland, Ohio, entered into a collaboration with medical technology provider Masimo to enhance hospital-based remote patient monitoring (RPM) services, such as TeleCritical Care.
