Canada's Ontario Government has initiated the construction of a new Reactivation Care Centre in Hamilton, which will offer specialised care for patients transitioning from hospital to community care.

The provincial government is investing more than C$16m ($11.6m) in this healthcare facility.

It is being developed in collaboration with St Joseph's Healthcare Hamilton and Hamilton Health Sciences.

The centre aims to provide the right care in the appropriate setting for individuals who no longer need acute hospital services.

The new 28,000ft² Reactivation Care Centre will feature 57 transitional care beds across three units.

These units will cater to a range of reactivation care needs, including dementia care services, complex care and behavioural support services.

In addition, the centre will house four rooms equipped for dialysis services, enhancing patient access to on-site haemodialysis treatment.

Ontario Deputy Premier and Health Minister Sylvia Jones said: “Our government is making historic investments in Ontario’s health care system to expand access to care and make it faster and easier for people to connect to the care they need, closer to home.

“The new Reactivation Care Centre will ensure more patients and their families in the Hamilton area are connected to the care and support they need, for years to come.”

Separately, the Ontario government is launching a new Youth Wellness Hub in Oxford County to improve access to mental health and substance use services for young people.

This addition is part of a broader initiative to expand the network to 32 hubs across the province.

Starting in 2025, the hub will offer free and convenient mental health, and primary care services to youth aged 12 to 25 years in a welcoming environment.

A mobile hub model will also serve rural communities, providing easier access to in-person counselling and peer support.

Both the Youth Wellness Hub and the Reactivation Care Centre are integral components of the Ontario government's 'Your Health: A Plan For Connected and Convenient Care', aiming to enhance healthcare accessibility and efficiency throughout the province.
