Monogram and Millennium team up to boost CKD outcomes

Monogram Health and Millennium Physician Group have joined forces to enhance the outcomes of chronic kidney disease (CKD) for several thousand patients in Florida.  

These value-based organisations aim to provide affordable care for individuals dealing with complex chronic conditions.  

This multi-year collaboration integrates Millennium's physicians and advanced providers with Monogram's extensive network of in-home nephrology and specialty care providers. 

This will lead to a care model aimed at enhancing outcomes for numerous patients throughout Florida. 

Under this partnership, Millennium's patients grappling with CKD, ESKD, and related chronic conditions will be referred to the programme.  

This referral grants them access to Monogram's suite of specialised clinical resources and support services, which include evidence-based clinical protocols, intricate case and disease management, utilisation management/quality assurance, and medication therapy management.
