Mount Sinai Medical Center in the US has announced a partnership with the virtual care management platform HealthSnap to support chronic disease management programmes.
The collaboration, which began in March 2024 with Remote Patient Monitoring (RPM), is currently operational at three Mount Sinai locations in South Florida.
There are plans to extend the service to 13 locations by the end of the year.
The partnership aims to expand Mount Sinai's in-house Chronic Care Management (CCM) programme, with a focus on delivering services to their Medicare patient population.
The CCM programme is set to launch across Mount Sinai Primary Care in July.
RPM and CCM initiatives are projected to support over 4,000 patients by the end of this year, with a long-term goal of assisting up to 10,000 patients with chronic conditions.
HealthSnap's platform is designed to streamline chronic condition management by enabling proactive care and maintaining patient engagement between clinic visits, which is expected to significantly improve patient outcomes.
Mount Sinai's adoption of HealthSnap's RPM and CCM programmes will initially support patients with a range of conditions including hypertension, diabetes, and congestive heart failure, among others.
The programmes are expected to expand to cover more conditions in the future.
Integration with EPIC, a leading electronic health record system, has concluded, providing Mount Sinai clinicians with a unified platform to collect, evaluate and act upon patient data.
This integration also streamlines the process for clinicians to file claims efficiently.

In December last year, Mount Sinai Medical Center and patient safety and compliance technology provider ObservSMART announced plans to enhance their strategic partnership to increase patient safety for behavioural health patients within three emergency centres across Miami-Dade County.
