Nascentia Health to leverage Biofourmis’s connected platform

The care-at-home programmes are aimed at achieving goals such as reducing unnecessary hospital readmissions.

Nascentia Health in the US has signed an agreement to leverage Biofourmis’s technology-enabled connected platform to launch care-at-home programmes.

Through this partnership, more older adult people across Central New York will be able to receive quality care from their homes.

The care-at-home programmes are aimed at achieving goals such as reducing adverse outcomes and unnecessary hospital readmissions.

Other goals include expanding care access, reducing average length-of-stay, improving post-acute care coordination, and enhancing outcomes and experiences of patients.

Additionally, patients will be supported in their transition from facility-based care to home by providing more care, tasks and monitoring. 

Nascentia Health chief clinical officer Andrea Lazarek-LaQuay said: “Leveraging Biofourmis’ unparalleled expertise, clinical knowledge, and cutting-edge connected platform, Nascentia Health aims to seamlessly integrate these advancements with our existing technology.

“The integration of Biofourmis' technology will enable Nascentia Health to broaden our spectrum of services, ensuring a superior level of patient-centric care is provided for our patients in the convenience of their own homes.”

Nascentia Health, which provides care management and insurance services, is currently taking part in a hospital-at-home programme with SUNY Upstate Medical University.

By adding the Biofourmis Care platform, it enhanced its hospital-at-home programme in early October.

The platform offers configurable, connected and personalised care options for managing patients virtually and in person using clinical-grade wearable devices, AI-guided algorithms approved by the US Food and Drug Administration (FDA), and more than 70 available care pathways.

In the coming weeks, Nascentia Health will seek engagements with other health systems and care provider organisations for launching care-at-home offerings.
