Oncology practice New York Cancer & Blood Specialists (NYCBS) has collaborated with Canopy, an intelligent care platform designed to support patients and caregivers across their cancer journey.
NYCBS operates value-based care initiatives with over 10,000 enrolled patients in programmes such as principal and chronic care management (PCM/CCM).
Through the implementation of the entire Canopy Platform, the practice looks to maximise the effectiveness of these programmes.
The platform assists practices in identifying and prioritising patients in need of assistance, resolving issues remotely using decision-support software, and creating new reimbursement possibilities.
NYCBS will initially leverage Canopy's programme management module to simplify the enrolment and workflow procedures for PCM and CCM, and use triage support tools to improve the efficiency of care delivery.
In addition, it also intends to integrate Canopy's electronic patient-reported outcomes (ePRO) system for monitoring symptoms remotely.
