PointClickCare partners with C3HIE to enhance patient care transitions in Texas

The partners aim to streamline care coordination and support safer patient transitions between healthcare facilities.

Canadian healthcare technology platform PointClickCare has partnered with healthcare data group C3HIE to enhance care transitions for patients across Texas.

The partnership aims to improve health outcomes and provide a connected care experience by integrating C3HIE's admission, discharge and transfer (ADT) data with PointClickCare's platform.

It will incorporate extensive Skilled Nursing Facility (SNF) data from PointClickCare into the C3HIE network in an effort to enhance healthcare interoperability in Texas.

This is expected to streamline care coordination and support safer patient transitions between healthcare facilities.

The partnership builds on a previous deal between PointClickCare and C3HIE, which included working with the Texas Health Services Authority (THSA) in 2022 to expand the Emergency Department Encounter Notification (EDEN) network.

PointClickCare Acute and Payer Markets senior vice-president and general manager Brian Drozdowicz said: “This collaboration is the latest example of PointClickCare’s efforts of improving care collaboration for individuals across the state of Texas, and we’re pleased to be working closer with C3HIE.

“We have a unique ability to provide this critical data directly, promptly, and reliably, something that is incredibly important when it comes to ensuring effective collaboration and enhanced care outcomes.”

PointClickCare's latest partnership with C3HIE aims to allow providers and care managers in Texas to receive real-time notifications, which has become more important as levels of hospital data in the state have increased.

C3HIE will add more than 40 hospitals to the PointClickCare platform to allow access to improved real-time data and insights, with the aim of furthering interoperability among care teams and improving Texas' overall healthcare system.

C3HIE CEO Phil Beckett said: “This partnership aligns with C3HIE's mission of making the healthcare community more connected and collaborative.

“By providing care teams with access to the necessary data, we empower them to make well-informed decisions together, leading to better health outcomes for the patients they serve.”
