Cloud-based health equity platform Spatially Health has partnered with Janus Healthcare Partners to enhance access to behavioural health services and address social determinants of health (SDOH).

Janus Healthcare offers an evidence-based model that integrates behavioural health services into primary care settings through the Collaborative Care Model (CoCM).

This evidence-based model, through telehealth, enables seamless access to psychiatric consult liaison services, psychotherapy, and medication management.

The partnership aims to tackle health-related social needs in patients of Spatially's clients by leveraging its Equity Equalizer platform.

Designed to identify and address individual health barriers, the platform can match patients to suitable social care solutions.

Janus Healthcare Partner's Samepage platform will further enhance this process, ensuring fidelity to the collaborative care model.

With custom-made clinical guidelines, Samepage includes a patient registry with longitudinal systemic outcomes tracking, clinical decision support, clinical task management, and clinical modules.

By leveraging data from Equity Equalizer platform alongside Samepage platform, the partnership intends to create tailored solutions for clients, ensuring they receive holistic care that meets both their physical and behavioral health needs.
