Vera Whole Health has entered a partnership with Anthem Blue Cross and Blue Shield in Connecticut, US, for the introduction of an advanced primary care model in Hartford.
This model aims to enhance the accessibility and quality of primary healthcare services in the area.
The advanced primary care model is designed to enhance the way members access and engage with healthcare, how providers deliver care, and the way employers finance healthcare services, the company noted.
The care centres of Vera Whole Health are placed as alternatives to standard healthcare systems and provide comprehensive care that addresses the whole person within a primary care environment.
This approach comprises using data-powered insights about the member and combining multi-functional care teams, including health coaching, behavioural health clinicians, and diabetes educators, into the primary care setting.
Vera also announced the opening of the new Vera Whole Health clinic at 399 Farmington Avenue in Farmington with a ribbon cutting ceremony.
Officials from Vera Whole Health, Anthem Blue Cross and Blue Shield in Connecticut and State of Connecticut Comptroller Sean Scanlon, among others, took part in the event.
The primary care solution can be integrated seamlessly with the benefits ecosystem and programmes of Anthem to boost health outcomes for members, cutting down the cost of care.
By combining technology with high-quality primary care, Vera Whole Health constructs a comprehensive health profile for each individual.
This profile is used to personalise outreach, particularly targeting members who stand to benefit most from high-quality primary care.
Once members are engaged, they are supported by a dedicated care team that includes primary care providers, behavioural health clinicians, health coaches, and Care Guides.
Vera Whole Health parent company apree health CEO Don Trigg said: “Our whole-health primary care model has industry-leading satisfaction, clinical quality and outcomes.
“We are excited to partner with Connecticut employers who want to help their employees better manage their health and care.”
