Health and community care technology company WellSky has entered into a new partnership with CareRing Health, a company with aims to enhance healthcare delivery for chronically ill, aged, and disabled individuals.

As part of this collaboration, CareRing is set to utilise WellSky's electronic health record (EHR) technology along with other analytics solutions and services to further optimise care for home-bound patients.

In addition, the partnership between WellSky and CareRing Health is designed to address the needs of the US's dually eligible patient population, who qualify for both Medicare and Medicaid.

CareRing Health's integrated care delivery model encompasses PrimaryCare@home, SkilledCare@home, PersonalCare@home, and HospiceCare@home, targeting chronic conditions such as hypertension, COPD, and diabetes.

This demographic, averaging 65 years of age, accounts for one-third of federal healthcare expenditure, the company noted.

By focusing on home care coordination, CareRing Health aims to transform the US healthcare system, managing chronic health conditions more effectively and preventing them from escalating into acute crises.

CareRing Health's approach is expected to reduce the necessity for emergency department visits, hospitalisations, and nursing home admissions, thereby alleviating the strain on both patients and healthcare systems.

The organisation has established Health Hubs in Pennsylvania and Delaware, with additional services across Ohio, Virginia, Maryland, and Washington, D.C.

CareRing Health CEO Steve Guenthner said: “There are approximately 13 million Americans who are dual eligible, and we find that for many of them, their needs are not being met by our healthcare system.

“There is a considerable need for integrated home care services for this population, who often have chronic health conditions and are traditionally underserved.”

CareRing stated that WellSky's technology will play a crucial role in coordinating care and offering insights to better identify patient needs and implement targeted interventions.

This collaboration marks a key step towards improving the quality of life for vulnerable populations while aiming to reduce the burden on national healthcare resources.

WellSky CEO Bill Miller said: “Smart technology gives healthcare providers a comprehensive picture of a patient’s overall health, including their medical, social, and behavioural needs.

“Advanced analytics, care coordination tools, and patient engagement capabilities can help to ensure that all individuals, no matter their background or socioeconomic status, have access to vital healthcare services.”
