Health and community care technology company WellSky has formed a new partnership with Vytalize Health, an Accountable Care Organisation (ACO), to enhance value-based care outcomes.  

Vytalize Health is a risk-bearing provider enablement platform, which addresses significant challenges in the healthcare industry with its care delivery system.  

It partners with more than 5,000 providers to elevate care quality through evidence-based medicine. 

Through the latest partnership, Vytalize Health will use WellSky's Next Generation Provider solution and services suite to improve care coordination and patient outcomes by supporting bi-directional communication between acute and post-acute providers. 

Leveraging the suite, Vytalize Health also aims to improve sharing of critical patient information across care settings, ensuring that patients are discharged to high-performing post-acute facilities.  

WellSky's solutions connect Vytalize Health's network of primary care physicians and specialists to an extensive network of more than 2,000 acute and 130,000 post-acute providers.  

This connectivity allows providers to identify Vytalize Health patients in their care and understand the benefits of its post-acute provider network.  

At discharge, hospital care teams can refer patients to the most appropriate post-acute providers for continued care. 

The partnership, which is already operational in several markets, also aims to enhance communication and collaboration between physicians.  

It provides critical insights into a patient's care journey, enabling providers to close care gaps, reduce unnecessary services and testing, prevent readmissions, and track patient locations and care timelines.
