US-based health management organisation ZeOmega has integrated its Social Determinants of Health (SDOH) Social Care solution with Microsoft's Cloud for Healthcare platform.
The combination of these technologies aims to increase the efficiency of health plans by automating workflows and providing a comprehensive view of care needs for populations.
By addressing social determinants of health such as transportation and food insecurity, the integration seeks to improve member and patient engagement, reduce expensive hospitalisations and optimise patient care.
It will allow healthcare professionals to access visualisations of social, clinical and behavioural information on one platform.
This is intended to empower case managers, clinicians and other professionals to make informed decisions and take appropriate actions within their existing workflows.

According to ZeOmega, the integration with Azure will ensure administrative efficiency, compatibility and access, ultimately allowing healthcare organisations to prioritise patient care and improve outcomes related to social determinants of health.
